Well, mostley I start surfing a bit. Collect some cool graphics. To come up with some idea's. Then start shuffling a bit etc. etc. And mostley some cool stuff come's out of it And my customers deliver most of the time some headers / background shizzle etc. For a example: http://www.dutchcovers.nl/.
