Extra Field When creating Thread

Hello,

I'm looking to know how can i create something similar to the attached picture. Its like an extra field with a checkbox that the user must pick up before posting.

I want to create something like "Please select this box in order to confirm you agree with our T.0.S."

I've tried already "SiteResort extra threadfields" hack, but it doesn't like exactly like this one.
