I'm looking for someone to create a custom mySQL database with ~15 fields. 2 of the fields in the database will be linked into my vBulletin data and the remaining fields for each record will be input by me.

The database needs to have a search function with it that will reside on the front page of our site...it needs to allow members that have registered for our vBulletin forum to search the database by multiple fields (ie "find all locations in this city that also sell this product") and also allow them to search by proximity (ie "give me all locations that serve a certain product and are within 50 miles of my current zip (which they'll have to input)).

Key attributes of the database we're looking for:

1. simple to maintain (since we'll be entering and maintaining a lot of data).
2. simple to search (so the members find it fun and easy to use).
3. fast (it doesn't have to be the fanciest architecture in the world - keep it simple...keep it fast).
4. Reliable - hopefully if its kept fairly simple, it'll be harder to break.
