Custom newthread Template

I need a template (for designated forums) that is a bit different than the normal title/message default. What I need it to show is below:

Artist(s):
Album:
Cover (Optional):
Tracklist (Optional):

I will need it to do things like automatically wrap IMG tags around the cover link, as well as CODE tags around the tracklist.

I don't know how long this will take, or how hard it is to do, so I'm willing to pay accordingly. Thanks.

Take a look at the recipe hack. You should be able to modify it to work well with this

Make sure you contact whitemike and see if it's OK with him for you to modify first ;)

I would, but I was looking at it, and it's not exactly what I want. I don't want to create tables above the actual thread. I want the info to actually be posted in the regular thread. Willing to pay anyone that can do it for me!
