I understand the user title setup, but how do I allow users to add a custom user title? Admins/mods have the option, but it doesn't show for a normal user.

There is an option somewhere (that naturally i cant find when i need to help you) to enable or disable custom user titles.

enabling it will allow users to set there own custom title.
Keep in mind the custom user title overrides the rank provided by the board member level titles (based on post count)

so if you have a rank system setup where the user is titled "n00b" for 0-10 posts, and the user sets their own custom title, it will no longer say "n00b"

You can set it per usergroup, of who can make custom titles and who can not.

Can you tell me how I can place user icons automatically under their name?

thats done with user ranks.... there is an option for it in ACP... there may also be hacks whichexpand on this feature as well.
