Well, you may or may not have seen my thread I posted about a week ago saying I needed a model application made, but I made one myself. Now I'm at a dead end on getting some things to work or even knowing how to do some things. The only things I have left to add so far are:

1. Multiple upload fields for adding attached images
2. Once this application is submitted, I need it to be emailed to me and another admin.

Could someone PLEASE help me out how to accomplish these tasks?

Do you want to store the uploaded files in the DB or as a physical file on the server?
