Help edditing, got stuck

ok i am trying to edit the login box to add a disclaimer...

as u see in the pictuer below i wanna make another login box with the border but with words(as it for my disclaimer) in it and i need it above the orignal Login box.

I can't seem to figure out what the problem is, perhaps you can enhance on this?
